Student COVID-19 Vaccination On-Site Clinic: Pre-registration Deadline 4/19

WCS will be hosting an on-site COVID-19 School Vaccination Clinic through the Clinton County Health Department. The clinic will be for students age 16 or older and will be run with the same safety protocols as other vaccination clinics run by the Health Department.

Clinic date/time to be determined, but will most likely be held during the school day.  We will notify you as soon as the clinic is scheduled.

Parental Consent for Vaccination of 16 and 17 Year Old Teens: ODH Updated Guidance

  • Children ages 16 and 17 who are not emancipated must have parental or legal guardian consent for any vaccine .
  • A parent or legal guardian should generally accompany the minor to receive the vaccine unless the vaccine administration  occurs in a physician’s office, school-based or school-associated clinic, or similar setting. In the specific settings noted, 16- and 17-year-olds may now be vaccinated without parental presence.
  • The vaccine will be Pfizer (EUA available below)

  • COVID-19 Vaccines are free

Students who are 18 years old, or an emancipated minor, may complete their own pre-registration and consent form.

Submit one form for each student. Download the additional Health Dept Registration/consent form or pick one up in the school office.

NOTE: Ohio does not mandate vaccination, nor does WCS. Our local Health Department has generously offered to hold an on-site clinic for the convenience of parents and students.
