Vaccine Clinic June 3rd - New opportunity for students and families

If you and/or your child (12 or older) have not yet received a COVID-19 vaccine and you are interested in starting that process, please read the following.

NEW! We have added a 1st shot clinic on June 3rd at 3:00pm in the WMS Cafeteria for kids 12 and older. Adult family members can also sign up to get a COVID-19 vaccine at this clinic!  The vaccine will be Pfizer.

  1. You must pre-register online here for the first shot or call (937)382-1641 before noon on June 3rd.

  2. An Additional consent form (pdf for printing) signed by a parent/guardian is required for children 12-17. Copies of the consent form are also available in the WMS office.

  3. Arrive by 3:00 pm on 6/3 and enter through WMS main entrance. You will be directed to the WMS cafeteria for the shot clinic.    

  4. This is a 2-dose vaccine.  There will be 2nd shot clinic on June 24th at 3:00 pm.  Please mark that on your calendars and watch for a follow-up email with more details closer to that date.                         

(If your child received their 1st shot at our clinic on May 13th, you will receive an email with instructions and an updated time for their 2nd shot clinic)

NOTE: Ohio does not mandate vaccination, nor does WCS. Our local Health Department has generously offered to hold an on-site clinic for the convenience of parents and students.
