Final Forms
FinalForms allows you to complete and e-sign enrollment, back-to-school, and athletic participation forms for your students. Here are some highlights:
- FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice!
- FinalForms also pre-populates information wherever possible, for each of your children, saving you time.
- You may review your data at any time to verify that it is current.
- You will be required to e-sign your forms once per year and after any update.

Getting Started
IMPORTANT: If your child is already enrolled in WCS, with a valid parent email address on file, then you should receive an email prompting you to confirm and complete your registration. Simply follow the steps below to get started:

Review the guide for Currently Enrolled Students if your child(ren) already attend WCS.

Follow the instructions in the email you receive to confirm and complete your account registration. (If you have a child that participated in athletics at WCS, then you will just need to log-in to your current FinalForms account, update children already in your account, and make sure you have added any additional children that will be attending WCS)

Add each child attending WCS - If this is your first time using FinalForms, you will then need to add each child that will be attending WCS this year. Enter the information requested for each child. Next year you'll only need to review and update.
Once you've completed the process, you can click here to access your Wilmington City Schools Final Forms account if you need to review/update any information.
Frequently Asked Questions:
What if I don’t have a computer or internet access?
No problem! If you don't have a computer, you can always complete them from a smart phone or tablet, accessing free wi-fi at a number of locations in Wilmington. If that isn't an option for you, there are computers available at the public library in Wilmington for public use. You can also contact one our building Principals and they’ll be happy to talk with you about other options.
How do I get started?
If your child is already enrolled in WCS with a valid parent email address on file, then you will receive an email on Aug. 1st prompting you to confirm and complete your registration. (If you have used Final Forms for Athletics this summer you may not receive an email. All you need to do is log back into your Final Forms account and complete the registration forms.)
What if I don’t receive an email?
If you do not receive an email, check your spam folder. If you still do not see the FinalForms email, please contact Janene Dunn in our Registration office at 937-382-7410 or janene.dunn@wilmington.k12.oh.us.
What if I don’t have an email address?
An email address is required for Final Forms. If you do not have an email address you can create a free email account with Google or any other email service you choose. Just contact a District Administrator and they can help you with that process, if necessary.
