5/13 COVID-19 Vaccine Opportunity for 12 and older!

On May 13th, WCS will be hosting an on-site COVID-19 Vaccination Clinic for students and staff through the Clinton County Health Department.

In anticipation of the Governor's guidance on opening eligibility for the Pfizer vaccine to children 12 years and older on Wednesday, we are allowing parents to pre-register their children ages 12 and older to get the 1st Pfizer shot at our clinic on May 13th. The clinic will be for students age 12 and older and will be run with the same safety protocols as other vaccination clinics run by the Health Department (pending anticipated ODH guidance).

Deadline to Pre-register: Thursday, 5/13 at 12:30pm

Submit one form for each student AND download the additional Health Dept. Registration/consent form (or pick one up in the school office).

  1. Pre-Registration Form (secure online form) -  Deadline is Thursday, 5/13, at 12:30pm
  2. Health Dept Consent Form (pdf) available from Cl. Co. Health Dept website and the school office. Sign and date the form "5/13/2021".
  3. 2nd Shot clinic will be held for these students on June 3, 2021.

NOTE: Students who are 12 to 17 years old who are not emancipated must have parental or legal guardian consent for any vaccine. 

Students who are 18 years old, or an emancipated minor, may complete their own WCS Pre-registration and Health Department consent form.

NOTE: Ohio does not mandate vaccination, nor does WCS. Our local Health Department has generously offered to hold an on-site clinic for the convenience of parents and students.
